She is survived by her sons, Michael Forman of Geneva and Dean Thomas of Childress, Texas; her daughters, Sandy (Brian) Keller of Auburn, Ga., Jan Watts of Austin, Texas, and Donna Fraley of Gorman, Texas; two sisters, Myrtle Milks of Candor, N.Y. and Lucille Smith of Rainelle, West Virginia; several grandchildren and several great-grandchildren and several nieces and nephews. She was predeceased by her husband Robert Forman. on July 31, 2013.³
