He was preceded in death by his father, Carl Ballard; and a niece, Sarah Ballard. He is survived by his mother, Lynn Miller; three siblings, Carla (Chris) Claytor, Christopher (Michelle) Ballard and Jesse Ballard; and uncle, John Mulquin; and several nieces and nephews, Tyler, Justus, Brianna, Danielle, Christian, Savanna, Ollie and Adaleigh.2 He was born on March 20, 1980 at Hardin Co., Kentucky.2,3 He died on August 22, 2021 at Radcliff, Hardin Co., Kentucky, at age 41.2
